Protocol for In-Office Visits
Hand washing
We recommend hand-washing before you enter the office for your appointment.
Hand sanitizer is also available throughout the office.
Screening
By entering the office you agree that you have self-screened for COVID-19 by answering 'no' the following questions:

Have you been exposed to anyone who has tested positive for COVID-19 within the past five days?

In the past 10 days?

Do you have any COVID-19 related symptoms?
Please contact us ahead of your appointment if you have answered 'yes' to any of these questions.
Additional Safety Measures
We require a medical-grade facial covering to be worn by everyone in the office including patients and staff for our mutual protection.
If you do not have a medical-grade facial covering, we will provide one to you.
